Identity and location fields

Use these fields when a form needs a structured name, address, location, or signature. The detailed task guides are:

Name

Use Name when you want name parts to be captured separately rather than asking for one free-text response.

Settings and structure

The Name field can include these parts:

• Title
• First name
• Initials
• Middle name
• Last name

Add only the parts that your process needs. The field lets you show additional parts directly while editing; each displayed part can have its own placeholder.

Guidance

• For a simple contact form, First name and Last name are usually sufficient.
• Do not require title, initials, or middle name unless there is a legitimate operational reason.
• Use neutral labels and avoid assumptions about name structure. A single free-text name field can be more respectful when the collected data does not need to be split.
• Test how names appear in notifications, exports, and workflows before relying on individual components.

Address

Use Address when you need a structured postal address.

Address settings

Setting	What it does	Guidance
Address style	Chooses Universal, United States, Canada, Australia, or United Kingdom address structure.	Choose the country-specific style only when your audience is limited to that country; use Universal for a broader audience.
Auto complete	Uses Google Maps to help complete an address.	Requires a Google Maps API key in user custom preferences. Enable only when that dependency is configured and privacy-reviewed.
Default country	Preselects a country in Universal mode.	Use only when it reflects most respondents and remains easy to change.
Allowed countries	Restricts the country choices in Universal mode.	Restrict only when you truly cannot serve other countries; explain the limitation elsewhere in the form.
API Key	Opens the user preference for a Google Maps API key.	Treat this as an account-level configuration, not a casual field setting.

Country restrictions and the default country apply in Universal address mode. Choosing a country-specific address style sets the address structure and associated country accordingly.

Privacy guidance

An address is personal data. Collect the minimum address detail required, make the reason clear, and do not include real home or delivery information in documentation screenshots or test submissions.

Map

Use Map to display a map or, when configured to save a value, allow a respondent to select a location.

Map settings

Setting	What it does
Interactive	Lets the respondent interact with the map.
Map Type	Lets the respondent switch among map views such as map, terrain, or satellite.
Zooming	Shows zoom controls.
Save Value	Lets the respondent drag the marker to an address and saves that location as a form response.
Full Screen	Lets the respondent expand the map.
Panning	Lets the respondent move around the map.
Rotation	Lets the respondent rotate the map where supported.
Street View	Makes Street View available where supported.
Search address	Lets the respondent search for an address and focuses the map on it. It requires a configured Google Maps API key.
Default zoom	Controls the initial detail level, from 1 (world view) to 15 (most detailed).

Only enable Save Value when you actually need a respondent’s chosen location. It changes the map from informational content into a data-collection field.

Signature

Use Signature when the respondent needs to draw a signature as part of the form.

Signature settings

Setting	What it does	Guidance
Pen color	Sets the signature drawing color.	Use a high-contrast color that is visible in exports and printouts.
Pen width	Sets stroke thickness.	Test on a touch device; thin lines can be difficult to create.
Required complexity	Requires a Simple, Medium, or High-complexity signature.	Use the lowest level that meets the business need, then test on mobile.
Required Field	Requires a signature before submission.	Use only after confirming that a drawn signature is the appropriate evidence for the process.

A drawn signature field does not by itself determine the legal validity of an agreement. If legal, identity, retention, or audit requirements matter, obtain appropriate policy and legal guidance before relying on it.

Test checklist

• [ ] The name or address structure fits the people completing the form.
• [ ] Country limits and defaults are intentional.
• [ ] Autocomplete and address search work with a correctly configured API key, if enabled.
• [ ] A map saves a location only when Save Value is enabled and intended.
• [ ] Signature capture is usable with touch and mouse input.
• [ ] Personal data is handled according to the organization’s privacy requirements.
