Survey and feedback fields
Use these fields when respondents are rating, scoring, or answering a structured set of questions. They are useful for feedback, satisfaction studies, quizzes, and simple evaluations.

Survey
Use Survey for multiple statements that share the same response scale, such as a Likert-scale satisfaction survey.
Settings

Do not mix positive and negative wording in the same survey without careful review. It makes responses harder to interpret and increases respondent error.
Slider
Use Slider for a numeric or ordered selection along a visible range.
Settings

Always include clear end labels. A bare 1–10 scale is too easy to interpret differently.
Net Promoter and Rating
Use Net Promoter for a recommendation-style score and Rating for a simple star-style evaluation.
Net Promoter
Net Promoter displays a fixed 1–10 score scale with an editable low-end and high-end label. Set the labels to make the meaning clear, for example Not at all likely and Extremely likely. A default score is available, but should normally be left unset to avoid biasing the response.
Rating
Rating displays a fixed five-star selection. Its field-specific behavior is intentionally simple: configure the question label, required state, custom error text, and appearance. It does not provide a separate field-specific configuration panel.
Keep the prompt specific. Instead of Rate us, write How would you rate the clarity of the installation instructions? Pair the score with an optional Long Text follow-up when you need to understand the reason behind it.
Quiz
Use Quiz to score answers from Single Choice, Multi Choice, or Drop down fields in the form.
Settings

The quiz setup uses choice fields already present in the form. Add and finalize the scored Single Choice, Multi Choice, or Drop down questions before assigning points.
Test checklist
- [ ] Survey statements and rating scales are balanced and unambiguous.
- [ ] Slider endpoints, range, step size, and default do not bias the result.
- [ ] Any price slider was tested through cart and payment.
- [ ] Quiz points match the intended correct answers.
- [ ] The pass threshold does not exceed the total available points.
- [ ] Timed quizzes clearly explain the time limit and auto-submit behavior.
